ent had had no spasm before that seen in thedispensary. Near the inner canthus of the risjht eye was a red, oedematousarea, the seat of the wound, now healed. There were no signs of suppura-tion of the wound, rendering it impossible to obtain material for makingcultures. He complained of no pain in the wound or elsewhere. The pupilswere normal and reacted readily both to light and distance, while the righteye could not be closed and was the seat of conjunctivitis. Trismus was bythis time so marked that the patient could open his mouth only about one-fourth the normal amount, and with difficulty could protrude his tongue,which was coated and at the edges bore evidences of having been bitten.The muscles about the neck were prominent and rigid, especially the posteriorgroup. The chest was well formed, and the head and lungs were normal. Theabdominal muscles and those of the back were hard and prominent; other-wise the abdomen was negative on examination. TETANUS CEPHALIC. 29 were normal, the prepuce was adherent, but was easily pushed back, andconsiderable smegma was removed. The temperature was normal and hisgeneral condition was good. The first night in the hospital the patient slept well, but the followingnight he was quite restless and cried out several times in his sleep. Threedays after admission, the eighth of the disease, the trismus was much moremarked, the muscles of the neck and back were stiffer, and a tendency toopisthotonos was first noticed. He then for the first time complained ofcontinuous pain in the back and legs, and he was quite rigid all over excepthis arms, which could be readily used, although his grip was weak and histhumbs presented a tendency to be drawn into the palms of the hands. Heresisted the most vigorous efforts to flex the legs on the thighs, but couldslowly flex them voluntarily when told to do so; the right leg was the morerigid.
